This is a state-certified program through the Colorado Division of Workers' Compensation that provides the holder with a 5-10% credit on their workers' compensation insurance premiums.
In the first hour, the presenter will cover the six steps of the program to explain exactly what type of documentation the State requires in order to be certified.  In the second hour, the presenter will be available to work with individuals who have brought their materials with them; she will go through your documents and help to assemble them.  You could leave this workshop with your entire application and supporting documents ready for submission to the State.

Materials Needed:  If you wish to assemble your materials during the workshop portion of the presentation, please bring the following items:

Written commitment to safety:  Usually found in the first part of your safety manual - it will state the company's commitment to safety i.e. "safety is our top priority," etc.  It must be signed and dated by upper management/owners.

Safety Coordinator or Committee:  This signed and dated document should name the person(s) responsible for safety and list their job responsibilities; conducts safety training, accident investigation, researches and discusses safety policies and objectives, identifies unsafe conditions and practices, establishes and updates safety rules.

Samples of Safety Training that have been Completed in Your Organization: Four to five examples of safety training.  These documents will have a date on them, employees have signed off on each one and they have a topic of the safety item discussed on the form (at least one of the samples has to have a date >1 year old).

Written Designation of Two Medical Providers:  You may find this posted on your bulletin board or you may have handed it out to employees.  Your agent or insurance carrier may also be able to put a written notice together on how long your company has had your providers, but the document must identify a date that shows you have had these providers in place for more than one year.

Written Claims Management Program:  On your company letterhead with a date showing your claims management has been in place for more than one year, this document would list the fact that you:
1) Investigate all incidents and document any third party involvement in the injury.
2) Contact your insurance company in a timely manner (8 hours, same shift, same day, 24 hours - whatever that standard is).
3) You meet with your carrier or agent annually to review claims/injuries, employee classification, experience modification factor, etc.
4) You offer modified duty to your injured workers whenever practicable.  You will need to prove this statement via a copy of a Rule 6 -Light Duty job offer you have made an employee in the past year.  We will have sample offer letters on hand for you to implement in your company if you have never used this administrative "Rule 6" in the past.

All items must be dated.  The date must show that each has been in place for more than one year/date more than one year old.
