Incident Investigation

Safety professionals are frequently called upon to conduct incident investigations when there is a loss in an organization. This session will introduce attendees to best practices in conducting an investigation that will identify causes of incidents in order to develop measures to prevent similar incidents from occurring again.

Attendees will learn to:

·        Understand the sources of error, with an emphasis on examining the system and not just the worker

·        Protect the investigation process to ensure that the information gathered will not be used by its adversaries.

·        Take steps to mitigate risks posed by the investigation to limit exposure to OSHA and potential third-party civil lawsuits.

·        Learn how information should be collected and shared, and guidelines for protecting the investigation through an “attorney work product privilege” arrangement. 


Risk Assessment Workshop

Safety professionals are frequently called upon identify hazards and determine the risks involved. Using a risk assessment as a key tool in managing occupational safety and health gives employers and businesses a means to be proactive in identifying hazards and taking action to remedy the problems before an incident occurs.
Learning objectives include:
• Learning the basic concept and terminology on risk assessments
• Understanding of who should participate in RA’s
• Completing a risk assessment
    1. Identify the hazards
    2. Identify who might be harmed and how
    3. Evaluate the risk – identify and decide on the safety and health risk control measures
    4. Record who is responsible for implementing control measures with timeframe
    5. Record finding and monitor completion of tasks
